Joint mobilization therapy is a manual therapy technique that is used to improve or manage musculoskeletal dysfunction. In this technique, the therapist apply different amount of force using skilled passive movements at different speed and amplitude to move or open the joint in order to decrease the pain , stiffness or swelling around the joint.
This technique mainly given to improve joint mobility , decrease pain and swelling generally occurs after trauma, injury or after any post operative procedures for example – TKR , post operative shoulder etc. It also helps in decreasing muscle spasm, tissue extensibilty, induce relaxation, improve blood circulation.
It is also used in frozen shoulder, osteoarthritis of joint , post fracture cases as it helps in increasing range of motion , improve movement restriction and achieving activities of daily living.
Joint mobilization is a manual therapy technique where a trained physio applies precise, passive movements to a specific joint. It focuses on arthrokinematics—tiny gliding motions between joint surfaces—rather than broader, voluntary osteokinematic movement
The technique is guided by Maitland’s grading scale, from Grade I (gentle, small amplitude) to Grade V (quick, thrusting)
The exact mechanisms remain partly theoretical. But several key effects are well‑recognized:
1. Neurophysiological Desensitization
Mobilization helps reduce tissue sensitivity around the joint, dampens pain signals, and calms muscle guarding.
2. Mechanical Stretching
By gently stretching the joint capsule and periarticular tissues, mobilization encourages return of normal glide and alleviates stiffness.
3. Fluid Dynamics & Circulation
Passive movement enhances synovial fluid distribution and blood flow, promoting lubrication, tissue nutrition, and joint health.
4. Structural Realignment
For misaligned or “locked” joints, grade-specific glides can help restore proper alignment and smooth joint mechanics.

Different grades activate different mechanoreceptors (e.g., Type I/II for pain modulation; Type III/IV for stretching; Grade V for GTO to inhibit muscle tone)
Joint mobilization is effective across a broad spectrum of musculoskeletal issues:
Post‑surgical rehab — joints recovering from replacement or fusion benefit from controlled mobilizations.
When done by trained professionals, joint mobilization is safe and well-tolerated, but care is essential.
Potential side effects include:
At DMPhysios, every joint mobilization session is personalized, evidence-based, and supportive. Here’s how we guide you from assessment to recovery:
1. Comprehensive Assessment
Our expert physios evaluate posture, movement, joint alignment, and pain levels to pinpoint dysfunction.
2. Treatment Planning
We decide which grade and type of mobilization suit your needs—whether it’s oscillatory, sustained, or thrust-based.
3. Prepping the Body
Gentle exercises or heat ensure tissues are ready, making the mobilization more effective and comfortable.
4. The Hands-On Session
You’ll relax in a comfortable position while the therapist uses precise hands-on techniques to target restricted joints.
5. Real-time Feedback
Your comfort and responses guide the intensity and technique throughout the session.
6. Home Exercise Integration
To maintain gains, we assign tailored exercises for strength, flexibility, and proprioception post-session.
7. Ongoing Review
We monitor your pain, mobility, and function, adjusting treatment to ensure steady progress.
